✔️ Pharmacology:
- Domperidone: Dopamine agonist that blocks dopamine receptors in the ChemoreceptorTrigger Zone (CTZ) in the stomach.
- The gastroprokinetic function is based on inhibiting dopamine receptors in the gastrointestinal tract, affecting motility.
- Essentially no effect on dopaminergic receptors in the brain due to poor penetration across the blood-brain barrier.
- Restores normal upper gastrointestinal motility and tone.
- Aids stomach emptying, improves antral and duodenal peristalsis, and modulates pylorus contraction.
✔️ Dosage & Administration:
- Domperidone should be taken 15-30 minutes before meals and, if necessary, before retiring.
- Usual oral dose:
- Adults: 10-20 mg (1-2 tablets or 10-20 ml suspension), every 6-8 hours daily. Maximum dose: 80 mg daily.
- Children: 2-4 ml suspension/10 kg body weight or 0.4-0.8 ml pediatric drops/10 kg body weight, every 6-8 hours daily.
- In dyspeptic symptoms:
- Adults: 10-20 mg every 6-8 hours daily.
- Children: 0.2-0.4 mg/kg every 6-8 hours daily.
- In acute and sub-acute conditions:
- Adults: 20 mg every 6-8 hours daily.
- Children: 0.2-0.4 mg/kg every 6-8 hours daily. (Maximum treatment period: 12 weeks).
- By rectum in suppositories:
- Adults (including elderly): 30-60 mg every 4-8 hours.
- Children: Maximum daily dose rectally is 30 mg for those weighing 10 to 25 kg. The dose may be divided throughout the day if necessary.
✔️ Interaction:
- Bromocriptine's hypoprolactemic impact may be reduced by domperidone.
- Antimuscarinics and opioid analgesics may counteract Domperidone's effect on GI function.
- Caution is advised when used with MAO (monoamine oxidase) inhibitors.
✔️ Contraindications:
- Known hypersensitivity to domperidone.
- Neonates.
- Gastrointestinal stimulation potentially harmful conditions: gastrointestinal hemorrhage, mechanical obstruction, or perforation.
- Patients with a prolactin-releasing pituitary tumor (prolactinoma).
✔️ Side Effects:
- Hyperprolactinemia (1.3%)
- Galactorrhea, breast enlargement, soreness, reduced libido.
- Common: Dry mouth, thirst, headache, nervousness, drowsiness, diarrhea, skin rash, itching.
- Extra-pyramidal reactions were seen in 0.05% of patients in clinical studies.
✔️ Pregnancy & Lactation:
- Not recommended during pregnancy; safety not established.
- Can help with galactorrhea and postpartum lactation.
- Secreted in breast milk in small amounts, considered not dangerous.
✔️ Precautions & Warnings:
- Caution in children due to the risk of extrapyramidal effects.
- Caution in patients with hepatic impairment due to high metabolism in the liver.
✔️ Storage Conditions:
- Store at or below 30°C, away from light and moisture.
- Keep out of reach of youngsters.
